We’ll help you review your funding, eligibility, and any grandfathered arrangements you may have under My Aged Care.
Our team will guide you through all required forms and liaise directly with My Aged Care and your current provider. You won’t need to chase documents or make multiple calls, we’ll do it for you.
We ensure a smooth transition behind the scenes, including your Referral Code reactivation and funding transfer, so there’s no disruption to your services.
Before your new services begin, we’ll introduce you to your Care Manager and support team. This early introduction helps build trust and ensures everyone understands your preferences, goals, and routines.
Your Care Manager is your dedicated contact, they’ll check in regularly, adjust your plan as your needs change, and make sure you’re always supported.
We coordinate your care so there’s no break between providers. Your support continues as normal, just with Nexia Aged Care’s added warmth, transparency, and attention to detail.
